If you are a resident of a care facility you will be able to develop a relationship with our Certified Nurse Practitioner and doctor.  They will be able to detect changes in your health status before problems develop.  Our team is on call 24 hours a day, seven days a week.  If you need services during off hours, your care will be managed by someone who knows you.

Capabilities

When you choose an August Health Services doctor you will also be assigned to a Certified Nurse Practitioner who works for August Health Services.  A Certified Nurse Practitioner is a registered nurse who has completed additional training and passed a rigorous test that allows the Certified Nurse Practitioner to be licensed and certified to provide services to patients under the supervision of a doctor.  What this means to you - the resident - is more personalized care.  The Certified Nurse Practitioner will get to know you and your family and, along with an August Health Services supervision physician, will make sure that your health care needs are met.

Capability 1
Management of resident's care closely to treat any change in condition without a transfer to the hospital.
Capability 2
If hospitalization is required, an August Health Services doctor will take care of you while you are there.  If you prefer to have your private physician care for you in the hospital, that can be arranged with return to our services upon readmission to the facility.
Capability 3
If you are sent home you can continue to see our August Health Services doctor in his or her own office.
