Introducing Shift Handoff, a new feature now available in August Health Care Track. Leveraging the power of August Intelligence, Shift Handoff automatically compiles the critical resident care details captured within Care Track during the previous shift into a concise, structured summary. No additional documentation required.
A digital summary built for continuity of care
The senior living landscape is shifting. Senior living communities care for residents with increasingly complex health needs. The average resident is 87 years old. More than 94% of assisted living residents have at least one chronic condition, and three-quarters manage two or three. And roughly 4 in 10 residents have been diagnosed with Alzheimer’s or another form of dementia. With residents requiring more complex, attentive care, the margin for missed details during shift transitions gets thinner every day.
Shift Handoff was designed with this reality in mind. It helps ensure that critical care details are never lost between shifts, so communities can protect continuity of care as resident needs grow more complex.
When an incoming caregiver begins their shift, Shift Handoff gives them a structured guide for their changeover conversation, organized into five sections:
- On Alert — Residents currently flagged for monitoring
- Out of the Community — Residents away from the property
- Notable Occurrences — Events documented during the previous shift
- Missing Routines — Routines that were not complete
- Exceptions — Exceptions flagged during care delivery
With documented care events from the previous shift surfaced automatically, caregiving teams can walk into every transition discussion with a shared starting point — spending less time piecing together what happened and more time talking through what residents need next.
Built to support your team, not replace your process
An important note: Shift Handoff does not replace the verbal handoff discussion. It strengthens it. Think of it as a shared, structured foundation that makes those conversations more focused, more thorough, and more grounded in the details that matter most.

How it works
Caregivers document resident care throughout their shift in Care Track, just as they already do. Shift Handoff then automatically organizes the most critical details into a summary. To view, simply click the book icon in the top right corner of the screen. When shifts transition, incoming and outgoing caregivers can review the information together. And the incoming shift has it readily available should they need to reference it during their shift.
