A resident presses their call button at 2 a.m. A caregiver arrives, helps them to the bathroom, and settles them back into bed. A few minutes later, the call is closed. On its own, it’s a small moment. It’s when those moments become a pattern that the care team takes notice.
However, spotting that pattern isn’t always so easy, as the data lives in the nurse call system, separate from the resident’s care plan, medications, and the notes that tell the rest of their story. The next phase of our integration with Sage brings those pieces together.
Every Call Is Part of the Story
The first phase of the integration ensures census data such as move-ins, move-outs, and level-of-care changes from August Health syncs automatically to Sage.
With Phase 2, completed nurse calls from Sage now flow back into August Health. Clinical teams can see each resident’s call trends in August Health Insights, right alongside the rest of their care. Over time, that history can help clinical teams spot changes earlier and have better-informed conversations about each resident’s care.
“A nurse call is one of the clearest signals of what a resident needs. When that signal lives alongside the rest of a resident’s care, clinical teams can see the full picture and act on it sooner. Connecting Sage with August Health through our bi-directional integration makes that possible for the communities we serve together,” says Scott Hougham, Head of Strategic Partnerships, Sage.
Care That Fits How Your Team Works
Nurse calls are one part of a resident’s day. Most of their care follows a routine: help getting dressed in the morning, a shower, a walk to breakfast. That care belongs in the same record as everything else.
Through this integration, nurse call data captured in Sage flows directly to August Health, alongside care documented in Care Track, August Health’s point-of-care app, so each resident’s day lives in one place.
For communities that prefer to document care within Sage, Sage Tasking brings scheduled care tasks from August Health together with unplanned care and nurse call alerts in the Sage app. Caregivers can mark care complete, note exceptions, and respond to calls in one place, and all of it flows back into August Health.
“Our customers decide how their teams work, and our job is to make the systems they choose work well together,” says Kerry Thurston, Head of Strategy & Partnerships at August Health. “A caregiver’s shift is one continuous job: the scheduled care and the call that interrupts it. Technology should treat it that way, and every part of that work should come back to a complete picture of the resident.”
